United Healthcare will pay up to $20 million to Arkansas, plus 35 other states because of state law violations. The Arkansas Insurance Department says the violations involve claims-payment services and Arkansas will receive more than $224,000. United Healthcare will also be implementing a three-year process-improvement plan that will include quarterly reviews and yearly benchmarks. If United Healthcare doesn't meet the yearly benchmarks they've agreed to, the company could face more fees of up to $20 million.
